The CCYS taps into several behavioral health indicators for school-aged children, including Risk and Protective factors, substance use/abuse, mental health and antisocial behaviors. On this page, you'll discover what information IS and IS NOT collected. All survey responses are collected anonymously and strictly used for research purposes.
What information is collected?
Gender
The participant will select a choice from the following: male or female.
Age
The participant will select a choice from the following: 10 or younger, 11, 12, 13, 14, 15, 16, 17, 18, and 19 or older.
School Grade
The participant will select a choice from the following: 6th, 7th, 8th, 9th, 10th, 11th, or 12th.
Race/Ethnicity
The participant will select a choice from the following: American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or Other Pacific Islander, White, or Unknown/Other.
Living Arrangements
The participant will select the choices that apply from the following: with mother, with stepmother, with father, with stepfather, with foster parent(s), with grandparent(s), with aunt, with uncle, with other adult(s), with brother(s), with stepbrother(s), with sister(s), with stepsister(s), and with other children.
What information is NOT collected?
Personally Identifiable Information (PII) is NOT collected on the survey. The survey does NOT collect, name, address, zip codes, social security number, student identification number, telephone number, or e-mail address.
